Howard County Service Coordination Vision
Through outstanding support planning and linking to resources, individuals with developmental disabilities in Howard County will have opportunities made available to lead productive and meaningful lives.
What is Service Coordination?
The four cornerstones of Service Coordination include Assessment to identify the person's needs and preferred lifestyle, Planning, with the person and the person's support network to develop a written plan, Coordination including arranging for supports outlined in the plan, developing and accessing of natural and community support systems, and providing advocacy, and Monitoring and Follow-up with persons served, team members, and providers to assess progress.
Who can get Howard County Service Coordination?
People who live in Howard County who have a developmental disability such as: Autism, Cerebral Palsy, Down Syndrome, Epilepsy, Congenital Disabilities, Severe Learning Disabilities, or Intellectual Disabilities.
This disability must have begun before the age of twenty-two, be likely to continue indefinitely, and result in problems with two or more of the following: living and working independently, written or verbal communication, self direction, learning, mobility, or self-care
